What does exclusion reinstatement mean?
Reinstatement is HHS-OIG's decision to restore an excluded individual's or entity's eligibility to participate in Federal health care programs, and it is not automatic once the exclusion period ends. HHS-OIG states that the individual or entity must apply for reinstatement and receive written notice from HHS-OIG that reinstatement has been granted.
Does a reinstated person stay on the LEIE?
No. HHS-OIG's exclusions FAQ states: “Individuals and entities who have been reinstated are removed from the LEIE.” A plain LEIE search therefore shows nothing for someone who was excluded and later reinstated; the current file does not keep a public record of the past exclusion.
What does “Clear with History” mean?
This is CFExclusion's own result label, not an HHS-OIG term. Clear with History means no current exclusion, but one existed and was later lifted — a materially different, more informative answer than a bare “clear.” CFExclusion keeps that history by also tracking HHS-OIG's monthly reinstatement supplements, which the bulletin and FAQ describe as posted monthly alongside the exclusion supplements.

Check a name
Looking for a specific name? Search the HHS-OIG LEIE, SAM.gov exclusions, and Medicaid exclusion lists from 41 states — free, with no signup for your first 3 searches a day.
A $25 report turns that search into a dated, signed record: proof this name was checked against all 43 sources — HHS-OIG LEIE, SAM.gov, and 41 state Medicaid lists — on a specific date, clear or not. See a sample report →
A single check is a snapshot: HHS-OIG replaces the LEIE around the middle of every month, so a result from last month no longer reflects who's on the list today. OIG's own guidance recommends screening your full workforce and vendors on that same monthly cycle — a best practice it states plainly, not a legal mandate.
